1. Employer Recordkeeping Packet
A cover log for tracking who on your team has completed which confined space course, and when a refresher is due. Keep alongside each employee's certificate of completion.

2. Site-Specific Training Addendum Template
Online coursework covers general OSHA-aligned content. This addendum documents the site-specific hazards, equipment, and procedures added on top of it, as required by 1926 Subpart AA / 1910.146.
3. Hands-On Competency Checklist
Use after online coursework to confirm the worker can actually perform the equipment tasks their role requires, not just answer questions about them.
- Dons and adjusts full-body harness correctly without assistance
- Operates gas monitor: power-on, bump test, and reads display correctly
- Demonstrates correct order of atmospheric testing (O2, LEL, toxics)
- Operates ventilation equipment used on this site
- Demonstrates correct entry permit completion
- Demonstrates non-entry retrieval system operation
- Demonstrates two-way communication procedure with attendant/supervisor
- States correct emergency/evacuation procedure from memory
4. Atmospheric Testing Practical Signoff Form
Documents that atmospheric testing was performed correctly and in the right order before entry.

5. PPE & Retrieval-Equipment Signoff Form
Confirms required PPE and retrieval equipment were inspected and are in service before entry.
- Full-body harness inspected, no cuts/fraying/damage
- Retrieval line / winch inspected and rated for the entrant's weight plus equipment
- Calibrated multi-gas monitor present and functioning
- Respiratory protection matches the hazard assessment for this space
- Communication equipment tested and functioning

6. Rescue-Plan Verification Checklist
Per 1926.1211, rescue capability must be verified and ready before entry begins. See our full rescue planning guide for more detail.
- Rescue method identified for this space (non-entry retrieval vs. entry rescue)
- Rescue provider identified by name (in-house team or contracted service)
- Rescue provider has confirmed it can respond to this specific space in time
- If relying on public emergency services, they've been contacted in advance and confirmed capability
- Rescue equipment on-site, inspected, and matched to this space's configuration
- Attendant knows how to summon rescue without entering the space
7. Retraining Trigger Checklist
OSHA doesn't set one universal refresher interval; retraining is required whenever any of the following occur. Use this log to track triggers as they happen.
- Job duties changed for the affected employee
- A new hazard was introduced to the confined space or the work
- An evaluation or incident showed training wasn't effective
- Equipment, procedures, or the permit system changed
- 36 months have elapsed since the last certificate (recommended interval)

8. Role-Specific Pre-Entry Checklists
Quick-reference checklists for each role to review immediately before entry. Not a substitute for full course training, listed here for on-site convenience.
Entrant
- Reviewed the entry permit and understands the hazards present
- Wearing required PPE and retrieval equipment correctly
- Knows the communication signal/schedule with the attendant
- Knows to exit immediately if ordered or if warning signs appear
Attendant
- Remains outside the space for the entire entry, no other duties
- Maintains continuous communication with the entrant(s)
- Monitors for unauthorized entry by others
- Knows how to summon rescue without entering the space
Entry Supervisor
- Verified atmospheric testing and acceptable entry conditions
- Confirmed all entrants/attendants are trained for their roles
- Confirmed rescue capability is verified and ready
- Signed and authorized the entry permit; will cancel it if conditions change
Competent Person
- Confirmed the space has been correctly identified and classified
- Confirmed the written permit program is current for this site
- Confirmed rescue and emergency arrangements are documented
- Reviewed recordkeeping for all personnel assigned to this entry
